Primary/intended use(s) of photo/video: Documentation of the perpetuation of the art of the sport of Hawaiian Outrigger Canoe training, rigging, paddling and racing.

I hereby grant Oahu Hawaiian Canoe Racing Association (OHCRA) and Lanikai Canoe Club (LCC) the right to use my/child's picture, photograph, and/or voice recording in any and all forms of media and in all manners including, but not limited to, composite or edited forms, for OHCRA and LCC’s use.

I hereby grant OHCRA and LCC the absolute right and permission to copyright and/or publish images captured through video, photo, or digital camera, of which I may be included in whole or part, for news release, art, advertising, trade, or any other lawful purpose whatsoever.

I hereby waive any right that I may have to inspect and/or approve the finished product or the copy that may be used in connection therewith, or the use to which it may be applied.

I hereby release, discharge, and agree to save OHCRA and LCC from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composition form, whether intentional or otherwise, that may occur or be produced in the making of said images, or in any processing tending towards the completion of the finished product.

I, the undersigned, am the applicant named above, aged 18 or older, or am the Legal Parent/Guardian of the minor child applying for membership named above. My (or my minor child's) participation in LCC events, programs, or activities of any kind, are conditioned upon my agreement to the following:

All members must adhere to all club rules as found in the operations manual.

Activities of LCC may be suspended or rescheduled due to circumstances outside of LCC's control.

LCC membership/activity fees are non-refundable, non-transferable, and not prorated.

I (or my child) will inspect the facilities and equipment prior to use and will notify a coach or other supervisor if any unsafe or improper condition is discovered, and will refuse to participate.

I acknowledge and fully understand that participation in LCC activities, events, races, practices, programs, or any activity (hereinafter "LCC Activities") is an inherently dangerous activity which involves risk of serious injury, including permanent disability, death, or other losses. Risk may arise from any number of circumstances including, but not limited to, the actions or inactions of the participant or other persons, the rules of play, the condition of the premises or equipment. Risk, whether known or unknown, reasonably forseeable or not, are inherent in paddling and LCC Activities. I understand and assume the risks.

I accept personal responsibility and assume the liability for any damages incurred associated with the risks of participation.

I release, waive, discharge and covenant not to sue the Lanikai Canoe Club, their respective administrators, directors, agents, coaches, and other volunteers or employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used during participation, (hereinafter "LCC Parties").

I assume all risks associated with losses or damages arising from my participation in LCC Activities, whether due to LCC Parties' negligence or otherwise.

THE UNDERSIGNED HAVING READ THE ABOVE WAIVER AND RELEASE, UNDERSTANDS THAT THEY HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING, AND SIGN VOLUNTARILY.
